A thoughtful beginning
for the next chapter.
We help prospective residents, families and referral partners explore whether the home’s support is the right fit.
Adults with
developmental disabilities.
The program is designed for adults ages 18–59 who are eligible for regional center services and whose assessed needs can be supported in this residential setting.
The program supports people with intellectual disabilities, autism, cerebral palsy, epilepsy or another regional-center-eligible condition, including daily living skill needs and significant behavioral support needs.
Admission is individualized. The review considers the person’s medical, mobility, supervision and behavioral needs, their preferences, the needs of people already living in the home and the facility’s approved scope.
Placement is subject to assessed fit and available capacity. Contact us to discuss current availability and the appropriate referral process. This website does not establish eligibility or guarantee admission.
Time to learn.
Space for questions.
The person considering the move is central to the conversation. Their choices and support needs guide the next step.
Make an initial connection
Call us or send a general inquiry. Your regional center service coordinator can participate in the referral conversation.
Review needs and information
We review the person’s IPP and relevant assessments, current support plans, health information and other requested records through an appropriate sharing process.
Visit and get acquainted
Where appropriate, a pre-placement visit gives the person and their supporters a chance to see the home, meet people and discuss daily life and preferences.
Prepare a person-centered transition
If the placement is appropriate, admission arrangements, agreements, support planning and staff preparation are coordinated with the resident and planning team.
A little more clarity.
Can a family contact you directly?
Yes. A family member, prospective resident or other supporter may call with a general question. Regional center eligibility, referral coordination and individual assessment are discussed as part of exploring placement.
Is a diagnosis enough to determine admission?
No. We consider the whole person, including strengths, goals, daily support, medical and mobility needs, behavioral supports and the home’s ability to provide appropriate care.
What records might be requested?
The review may include the current IPP and progress information, medical and functional assessments, existing behavioral support plans, emergency contacts and other relevant documents. Please wait for instructions about an appropriate way to share private records.
What does the program cost?
Contact the administrator and your regional center service coordinator to discuss the placement arrangement, any applicable agreements and individual financial responsibilities. No rates or insurance acceptance are represented on this website.
Are openings guaranteed?
No. Availability and suitability are confirmed individually. Please call for current information.
A conversation is a good place to start.
For yourself, a family member or someone you support, we’re here to talk through the next step.
